Please download and complete the patient questionnaire before you arrive at our office and bring the items listed below. If you have questions about what to bring or the information on the questionnaire, please call us.
Please arrive 10-15 minutes early for your appointment for registration and to fill in a health questionnaire.
When you come for your Orthopedics and Sports Medicine Appointments, please remember to bring the following:
- Driver’s license or State ID
- Insurance information
- Copies of operation records, medical records, x-rays, MRIs, CT scans and so on from prior doctor visits
- Medications list (if any)
- If you have had surgery elsewhere, please bring a copy of your operation report
- Very often during the course of your evaluation or treatment for an orthopedic condition, we will require x-rays to determine the cause of a problem or to evaluate your progress.
To Cancel an Orthopedics Appointment or Sports Medicine Appointment
Call our office during business hours and allow at least one day’s notice so that we can offer your appointment time to patients on our waiting list.
We recognize that your time is valuable, and we make every effort to run on time. Occasionally emergencies or patients require a little more time, and these cause scheduling delays beyond our control. We apologize if we keep you waiting.

We take great pride in the quality of the care we give our patients and in our efforts to help them make informed decisions about their injury or condition and their treatment options. This website is part of that education effort and we have information and patient education videos in many areas of orthopedics and sports medicine; including information about injuries and treatments for injuries of the shoulders, elbows, knees and hands.
Dr. J. Michael Bennett is a Sugar Land Board Certified Orthopedic Surgeon and Sports Medicine Physician with offices in Houston in addition to Sugar Land. He grew up in the Houston area and graduated from the University of Texas. After completing medical school at the University of Louisville, Dr. Bennett returned to Houston for a residency in general surgery at the Baylor College of Medicine, followed by an orthopedic surgery residency at Baylor.
An athlete himself, Dr. Bennett was accepted to the prestigious UHZ Sports Medicine Institute in Miami for a Sports Medicine Fellowship. During that time, we served as an assistant team doctor for the Tampa Bay Buccaneers and the University of Miami Hurricanes. As evidence of his expertise in Sports Medicine, he’s earned a Certificate of Added Qualification as a Sports Medicine Physician.
